Pentalpha Lodge of Free and Accepted Masons No. 564 is located in the Indiana Freemasons' Hall at 525 North Illinois Street in Indianapolis, IN. The Lodge room is on the 5th Floor, South, in the Composite Room.

Stated Meetings are held on the first Thursday of each month, except July and August at 7:00 P.M. When the First Thursday falls on a legal holiday, the stated meeting for that month shall be held on the Second Thursday of the month at 7:00 P.M.

Pentalpha Lodge #564 is a Blue Lodge or Symbolic Lodge and is governed by the Grand Lodge of Indiana.
